Undergraduate student midwives at De Montfort University have a number of documents that need to be completed in practice
- Clinical assessment document: This contains competencies to be achieved by the student. There is one clinical assessment document for the each placement – below is an example
- Ongoing Achievement Record (OAR): This book is held by the student for the total length of their course, at the end of each placement this is filled in
- Clinical skills log: This contains the student’s record of skills that are required in order to complete the course
- Service user questionnaire these are for the practice supervisor to give to the women the student has cared for, to assess the student
- EEC booklet This is the student’s record of their EEC (EU) directives that they need to achieve to complete the programme
If you need clarification on filling out the above documentation, please contact either the link lecturer for your area or the academic assessor of the student concerned
