This information below provides a general overview of student nurse documentation.
Undergraduate student nurses at De Montfort University have a number of books that need to be completed in practice.
- Practice Assessment Document (PAD): This contains the competencies to be achieved by the student. There is one PAD for each year of the programme.
- Ongoing Achievement Record (OAR Book): This book is held by the student for the whole length of their training.
- Clinical Skills Log
- Service user questionnaire
- Inter- professional working student diary
As booklets are reviewed and altered on a regular basis, copies are not available on this website.
Recent changes have been made to reflect the roles of Practice Assessor and Academic Assessor. Details of where the changes have been made can be found in the document below.

Please be aware that all booklets/guides below were written pre- SSSA standards. Where mentor is written, this will be the Practice Assessor (see guide above for details).

Nurseries and pre-schools who have agreed to take a student from the child field of practice may find the Practice Assessment Documents that have been populated with examples useful. Please be aware that the format may have changed from the student’s physical copy as alterations to signing pages have been made.
